B-1 ACCIDENT INVESTIGATION REPORT RELEASED

An Air Force Globe Strike Command accident investigation board completed its investigation into a 20 April 2022 B-1B Lancer aircraft mishap at Dyess Air Force Base, Texas.

The accident investigation board found that the B-1's high cycle fatigue initiated a crack on the surface of the #1 engine's 2nd stage fan disk resulting in severed fuel lines which caused a fire. Debris from the explosion struck one Airman who suffered minor injuries and was treated promptly at the local hospital.
